To register, look at the upper right hand corner of this page where it says Login to Schedule an Appointment. Click on "here" at the bottom of this section where it says "Not a Member? Click here to register". You will then be connected to a link where you can provide your contact information. You will be contacted once we have received and reviewed this information.
